For simple screening for anxiety disorders, the 7-item Generalized Anxiety Questionnaire is commonly used in mobile health applications.
This mobile apps provides a sample implementation of the standard GAD-7 questionnaire.

MyAndroid is not a downloader online for GAD7 Questionnaire. It only allows to test online GAD7 Questionnaire with apkid com.mobiletechnologylab.anxiety.questionnaire. MyAndroid provides the official Google Play Store to run GAD7 Questionnaire online.
