HealthRise Transition Grant is being implemented to demonstrate
improved clinical outcomes of patients with Diabetes Mellitus and
Hypertension exposed to SALT-CLCP approach in Shimla Himachal Pradesh
and Udaipur Rajasthan.
Concurrent evaluation of the projects planned
activities will be undertaken by establishing a robust MIS, ensuring
rigorous monitoring process and on-ground supportive supervision to
FLHWs.
Reports generated from MIS will support data driven decision
making.
To facilitate the process, multiple templates/forms will be
developed to capture planned activities.
The various formats are-
1.
Village Profile- Each intervention village details will be
filled in using this format
2.
Baseline Assessment for the each registered Patient: This form
will record data related to current status of the patient in terms of
adherence to medication, lifestyle modification, clinical values of
diabetes and hypertension.
3.
Follow-Up Formats- This form will record follow-up data
related to status of the patient in terms of adherence to medication,
lifestyle modification, clinical values of diabetes and hypertension
on a quarterly basis
4.
SALT Group Meeting- This will be used to record details of
every SALT-CLCP Group Meeting across the intervention geography.
