DataBiologics uses standardized functional measurement surveys for eight different body parts and six different procedures including PRP, Lipogems and Prolotherapy.

Patients fill out this forms using our application on the date of their procedure and all answers are submitted to our database server with a specific patient ID.

Our setup automatically sends patients surveys at 3, 6, 12, and 24 month time periods after their procedure for follow-up data.
