The Healthcare Effectiveness Data and Information Set (HEDIS) is a tool used by more than 90 percent of health plans in the United States to measure performance on important dimensions of care and service.
The goal of every health plan is to obtain a 5 Star rating in their Commercial, Medicaid and Medicare lines of business.
To reach this goal their medical providers must be 5 Star providers.
This is achieved by obtaining the appropriate coding via claims submitted to the health plan and the results of the annual medical record review that occurs from January through May.
When a provider submits a claim or an encounter to a health plan the calculation of this data is called the administrative process.
When the health plan performs the annual HEDIS® medical record review audit this is called the hybrid process.
it is where a Random Sample of members are selected and medical records are collected at the medical providers offices.

This process is very disruptive to medical providers.
In order to avoid or reduce the number of medical record reviews providers should send the information that meets the criteria via claims data.
