【 How to Use this App 】 (Usage Instructions)
1. Input the User’s ID and the Password to Log in.
2. Fill out your name in the blank on “21 Items of Lifestyle Habit Check Sheet”.
3. Answer each of all 21 questions and hit the Diagnostic Result button.
※ You cannot view the diagnostic result unless you answer all 21 questions.

“11-Food Risk Analysis Sheet” (Result) is displayed on the screen.
※ The detailed explanation for the top three risks out of 11 risks is displayed.
※ By pressing “Confirm the Check Sheet” at the bottom left of the screen, you will be able to view the answers you have input on the “21-item Lifestyle Habit Check Sheet”.

By pressing “Download” at the bottom left of the screen, in IOS device, 11-Food Risk Analysis Sheet is converted to PDF and can be downloaded to the device.
※ For Android device, please open the PDF through the filer application such as “File Commander” or through Google Drive.
※ If download cannot be completed within 20 seconds, it will end up as an error.
